Resumo: Introdução: A gestação após bypass gástrico em Y-de-Roux (BGYR) parece ser mais segura do que quando ocorre na obesidade, porém, recomenda-se evitar a gestação nos primeiros 12 a 18 meses de pós-operatório, devido aos possíveis riscos provenientes da rápida e significativa perda de peso neste período. Ainda não , -se um menor ganho de peso gestacional entre essas mulheres e uma maior prevalência de bebês nascidos pequenos para a idade gestacional (PIG). O objetivo deste estudo é investigar o perfil clínico-nutricional do binômio materno-fetal após BGYR. Métodos: Estudo observacional, retrospectivo e analítico. Foram convidadas todas as mulheres operadas por BGYR, em um serviço privado de cirurgia bariátrica do Distrito Federal em funcionamento desde 2005, que responderam questionário virtual. Foram excluídas da amostra as mulheres que se submeteram a outras técnicas cirúrgicas, que engravidaram artificialmente ou apresentaram gestação múltipla, gestações interrompidas por aborto e portadoras de diabetes mellitus antes da cirurgia. Para a classificação do estado nutricional e ganho ponderal gestacional, utilizou-se a recomendação do Institute of Medicine (IOM - 2009). Os dados foram analisados no SPSS® versão 23. Para comparação das variáveis continuas, aplicou-se o teste de correlação de Pearson e para categóricas, aplicou-se o teste qui-quadrado. O nível de significância foi p<0,05. Resultados: Entre 30 mulheres elegíveis, 1 foi excluída por ser diabética. A média de idade entre as 29 mulheres estudadas, foi de 34,93 ± 4,32 anos (25-42 anos) e o IMC pré-operatório de 40,79±4,57 Kg/m² (33,71-56,65 Kg/m²). Em relação aos dados sociodemográficos, a grande maioria era casada, possuía ensino superior completo e trabalhava no setor privado. Quando engravidaram, apresentavam, em média, 43,48±30,66 meses de cirurgia e 72,93±15,45% (45,74-99,12%) de percentual de perda do excesso de peso (%PEP). Verificou-se que 72,41% das mulheres realizaram acompanhamento nutricional durante a gestação e, apenas 2 não utilizaram suplementação durante a gravidez. Observou-se a presença de complicações durante a gravidez em 13 mulheres (44,83%), no qual as mais prevalentes foram enjoos, náuseas e tonturas. Em relação as complicações nas crianças, observou-se que 1 delas apresentou pé torto congênito bilateral ao nascer. Demais resultados encontram-se no estudo. Conclusão: Na amostra estudada, a maioria das gestantes engravidou após os 12 meses de cirurgia e, apesar da média de ganho de peso gestacional aceitável, a maioria apresentou ganho de peso gestacional acima ou abaixo da recomendação adotada pelo estudo. A maior parte dos bebês nasceu a termo e, apesar de quase metade ter nascido com peso insuficiente, a maioria nasceu proporcional. O acompanhamento nutricional e a utilização de suplementação durante a gestação foram feitos por quase todas as gestantes. Houve associações entre: quanto menor o peso pré-gestacional, menor o peso da criança ao nascer; quanto menor o IMC antes da gestação, menor a estatura de seu filho e quanto maior o tempo entre a cirurgia e a DUM, maior o ganho de peso gestacional. Ressalta-se que os valores analisados tiveram uma correlação fraca, mas estatisticamente significantes. O teste de qui quadrado não mostrou nenhuma associação significante entre realizar ou não acompanhamento nutricional no período gestacional e ter um filho PIG. A gestação após BGYR parece ser segura, porém, há a necessidade de estabelecer parâmetros de referência específicos para o acompanhamento de gestantes bariátricas e seus bebês.
Abstract: Background: Pregnancy after a Roux-en-Y Gastric Bypass (RYGBP) seems to be safer than when it occurs in obesity, however, it is recommended to avoid pregnancy in the first 12 to 18 months postoperative period, due to the possible risks arising from the quick and significant weight loss in this phase. Still there aren't any specific parameters for weight gain during pregnancy or consensus on nutritional management of pregant women after bariatric surgery, but it is suggested a lower weight gain during pregnancy among these women and a higher prevalence of small for gestational age infants (SGA). The aim of this study is to investigate the clinical-nutritional profile of the maternal-fetal binomial after RYGBP. Methods: Observational, retrospective and analytical study. All women who had undergone RYGBP at a private bariatric surgery clinic in Distrito Federal, which is in operation since 2005, were invited to respond to an online questionnaire. Women who underwent other surgical techniques, got pregnant artificially or had multiple gestations, gestation interrupted by abortion and diabetes mellitus patients were excluded from the sample. The recommendation of the Institute of Medicine (IOM - 2009) was used to classify nutritional status and gestational weight gain. Data were analyzed in SPSS® version 23. For comparison of the continuous variables, the Pearson correlation test was applied, and for the categorical variables, the chi-square test was used. The level of significance was p <0.05. Results: Among 30 eligible women, 1 was excluded for being diabetic. The mean age among the 29 studied women was 34,93 ± 4,32 years (25-42 years) and the preoperative body mass index (BMI) was 40,79±4,57 Kg/m² (33,71-56,65 Kg/m²). Regarding sociodemographic data, the vast majority were married, had completed higher education and worked in the private sector. When they got pregnant, they had, on average, 43.48 ± 30.66 months of surgery and 72,93±15,45% (45,74-99,12%) percentage loss of excess weight (EWL%). It was verified that 72.41% underwent nutritional monitoring during pregnancy and only 2 didn't used supplementation during this period. The presence of complications during pregnancy was observed in 13 women (44.83%), in which the most prevalent were nausea, sickness and dizziness. Regarding the complications in children, it was observed that 1 had bilateral congenital crooked foot at birth. Other results are found in the study. Conclusions: In the studied sample, the majority of pregnant women became pregnant after 12 months of surgery and, despite the average of acceptable gestational weight gain, most had gestational weight gain above or below the recommendation adopted by the study. Most infants were born full-term, and although nearly half were born underweight, most were born well-proportioned. The nutritional monitoring and the use of supplementation during pregnancy were performed by almost all pregnant women. There were associations between: the lower the pre-gestational weight, the lower the child's weight at birth; the lower the BMI before gestation, the shorter the height of the child and the longer the time between surgery and the day of last menstrual period (LMP), the greater the gestational weight gain. It should be noted that the analyzed values had a weak but statistically significant correlation. The chi-square test did not show any significant association between whether or not to perform nutritional monitoring during the gestational period and to have an SGA child. Pregnancy after RYGBP seems to be safe, however, there is a need to establish specific reference parameters for the follow-up of bariatric pregnant women and their child.
